Project Summary
It is the partnership’s continuing vision to develop a comprehensive seamless continuum of care for our aging citizens. The CPOA grant project will produce a road map that will guide us in establishing a delivery system of long term care and supportive services which will serve our county now and into the future. There are three key components to the project.First, an in-depth five-year strategic plan will be completed. Development of the plan will entail assimilating and reviewing existing data regarding current services and demographics; obtaining community input regarding needs and preferences; compiling data in a usable format; prioritizing needs; developing goals and objectives for meeting the needs; creating a work plan and timeline for action; and creating an evaluation tool for monitoring purposes. The end product will be an in-depth strategic plan that will serve as a base from which to initiate positive changes to the long term care and supportive services delivery system.Second, we will develop and initiate an education component for both consumers and service providers. We will educate the community regarding long term care options. We will provide educational workshops and seminars on topics that impact seniors and expand the resource newspaper, “The MARC Magazine”. We will produce an in-depth resource directory to enhance access to services. Finally, we will develop a marketing strategy to create community awareness regarding the educational resources available.Third, we will strengthen the partnership in order to implement positive changes in the long term care and supportive services delivery system. Future accomplishments will be directly impacted by the strength of the partnership; it is imperative that we maximize the partnership’s potential. We will create a formal statement of the partnership’s mission, goals, and objectives. We will develop and define organizational processes. We will recruit for diversity, hold regular meetings, and recognize accomplishments. We will work on joint projects and write grants collaboratively, while allowing members to specialize in their areas of expertise and interest.
